Out of Warranty (OOW) Repair Request Form

This form is for registered Partners. Please fill out the form to repair Avigilon products that are no longer under warranty.

Once the form is completed, an OOW Repair Request is opened, and a confirmation will be emailed to the requester to print and ship with the product(s) to be repaired.

Important notes:

  • A US$100 evaluation fee applies to all requests.
  • If the Repair Request is authorized, the evaluation fee will be applied towards repair costs.
  • Items returned without first making a Repair Request will not be accepted.
  • Submitting an incomplete request will delay the Repair Request process.

CLEARLY INDICATE REPAIR REQUEST # ON THE OUTSIDE OF THE BOX.

Remove any lenses or other equipment not associated with the repair before sending. Any items sent to Avigilon that are not authorized by the Repair Request will NOT be returned.
