Once the form is completed, an OOW Repair Request is opened, and a confirmation will be emailed to the requester to print and ship with the product(s) to be repaired.
Please group your return request by the product’s model number. If there

CLEARLY INDICATE REPAIR REQUEST # ON THE OUTSIDE OF THE BOX.
Remove any lenses or other equipment not associated with the repair before sending. Any items sent to Avigilon that are not authorized by the Repair Request will NOT be returned.
